Clinical Decision
- Monthly case reviews from the facilities especially patient failing 2nd line and 3rd line regimen
- Identify active facility MDTs strengthen and formation where they are not existence
- Coordination with national hub on 3rd line drug switches
- Reports from Facility MDTs
- Phone calls from the facility teams consulting on different aspects of patient care and treatment
- Coordinating of the referral of cases that require further intervention e.g by consulting Nascop, specialists on behalf of the facility teams.
- Following up patients discussed by the TWG to ensure implementation of the interventions is done.
Indications for Nairobi HIV TWG Consult
- Patients failing 2nd and 3rd line ART treatment
- Patients failing PI based 1st line ART treatment e.g. CALHIV
- Patients failing INSTI (e.g. DTG, RAL) based regimen
- Patients with multiple comorbidities e.g. due to pill burden concerns, Drug to drug interactions.
- Patients with suspected ART ADRs not solved at the facility MDT level.
- Patients on non-standard regimens for standardization
- Patients with massive Adherence and psychosocial concerns
